Data Protection
Under the Data Protection Act 2018 (GDPR), I am required to advise all client(s) of my Data Protection Policy. As part of the Client Record keeping, this clinic is required to retain information which may be in paper or electronic form. This includes but is not limited to: information from consultations including medical history, treatment details and contact details. Information can be shared with third party medical practitioners only.
Any information shared with third party medical practitioners will be at the request of the client and must be formally requested in writing. All paper/electronic files will be stored for a maximum of 7 years at the SN Clinic, this includes but is not limited to: all client details, data protection declarations and consent forms. Once 7 years have passed all details will be securely destroyed in accordance with the Data Protection Act 2018. All information provided will be treated as confidential and will not be disclosed to any other person(s)/organisation(s) without written consent from the client concerned. Information will be held both manually and at times electronically in files accessible only by authorised personnel. If any photos are taken for social marketing purposes then verbal or written consent is required.
Any photos taken will be stored in a device only accessible by authorised personnel and will not be shared, shown or disclosed to any other person(s)/organisation(s) without written consent from the client.
Appointment Cancellation & Booking Policy
If you are unable to attend your scheduled appointment, please notify The SN Clinic at least 24 hours of your appointment to avoid the cancellation fee. Cancellations or requests to reschedule made less than 24 hours before the scheduled appointment time will result in the full appointment fee being charged. Any deposit paid will also be forfeited. All appointment payments and deposits are non-refundable. However, payments may be transferred to an alternative appointment date, provided that the request to reschedule is made at least 24 hours before the original appointment time.
Late Arrivals
The SN Clinic offers a 10-minute grace period for late arrivals. If you arrive more than 10 minutes late and have not contacted the clinic in advance, your appointment will be cancelled and the full appointment fee will remain payable, and any deposit paid will be forfeited
Outstanding Payments
The SN Clinic reserves the right to cancel or decline any online bookings where there are outstanding payments or unpaid fees associated with previous appointments. We kindly ask that all clients respect their scheduled appointments and provide as much notice as possible if they need to cancel or reschedule
Refund Policy
The SN Clinic does not offer refunds on appointment bookings or deposits. All payments are non-refundable and are subject to the terms outlined above.
