Data Protection
Under the Data Protection Act 2018 (GDPR), I am required to advise all client(s) of my Data Protection Policy. As part of the Client Record keeping, this clinic is required to retain information which may be in paper or electronic form. This includes but is not limited to: information from consultations including medical history, treatment details and contact details. Information can be shared with third party medical practitioners only.
Any information shared with third party medical practitioners will be at the request of the client and must be formally requested in writing. All paper/electronic files will be stored for a maximum of 7 years at the SN Clinic, this includes but is not limited to: all client details, data protection declarations and consent forms. Once 7 years have passed all details will be securely destroyed in accordance with the Data Protection Act 2018. All information provided will be treated as confidential and will not be disclosed to any other person(s)/organisation(s) without written consent from the client concerned. Information will be held both manually and at times electronically in files accessible only by authorised personnel. If any photos are taken for social marketing purposes then verbal or written consent is required.
Any photos taken will be stored in a device only accessible by authorised personnel and will not be shared, shown or disclosed to any other person(s)/organisation(s) without written consent from the client.
Cancellation Policy
If you are unable to attend your scheduled appointment, please notify the SN Clinic at least 24 hours in advance to avoid a full appointment fee charge. This fee must be paid before any future appointments can be scheduled. This policy applies to missed appointments, no-shows, and late arrivals, which result in session cancellations.
The SN Clinic offers a 15-minute grace period for late arrivals, provided no prior notice is given. If you arrive more than 15 minutes late without notifying the clinic, your appointment will be cancelled, and the full fee will be required.
Additionally, the SN Clinic reserves the right to cancel any online bookings if there are outstanding payments. We ask that all appointments be respected to ensure efficient scheduling for everyone.
Please understand that if you do not cancel or reschedule within the 24-hour cancellation period prior to the appointment, you will be charged the full amount of the appointment fee. Please also understand that this policy applies if you do not show up for your scheduled appointment or if you arrive late, resulting in the appointment being cancelled. SN Clinic can refuse any future bookings if an outstanding payment is not made.
